Transition Improvement Grant (TIG)
The Transition Improvement Grant (TIG) is intended to provide effective, targeted, and low cost professional development to Wisconsin youth, parents, educators and stakeholders in the area of postsecondary transition for students with disabilities. The Transition Improvement Grant will encourage effective practice Postsecondary Transition Plan (PTP) development, with a goal of improving post high school outcomes. This Grant will also focus on providing technical assistance to schools and LEA’s on data collection and reporting requirements of state performance plan (SPP) Indicators 13 and 14. The grant will utilize data collected to identify the local needs of school districts, communities and/or counties to provide improvement strategies that will positively affect the post school outcomes of students with disabilities. The CESA 4 Transition Consultant assists local school district personnel in identifying and implementing transition options for all students with disabilities in middle and high schools. The transition consultant serves as liaison between schools, community, agencies, human services, and post-secondary education to identify and develop appropriate transition plans throughout the IEP process. The district transition consultant provides on-site consultations as purchased per each special education program/teacher. The transition consultant is also available to assist in planning and developing work experience and vocational experience models for students.
County level consultation involves regular meetings between school districts, state, county, and community agencies to plan, coordinate, and support transition for students with disabilities from school-to-work, independent living, and post-secondary education.
